Action Steps:
Charge the Hartford Commission on Homelessness with creating an implementation plan.
Measurable Outcome: The development of an implementation plan within 90 days of the time it is launched by the Hartford Commission on Homelessness. The implementation plan will address the following components:
- Describes specific actions to be taken.
- Assigns responsibility for each action.
- Suggests the timing by when key actions are to be taken and completed.
- Defines anticipated outcomes and results of successful efforts.
- Identifies the level and proposed sources of the funding and resources needed to implement each action.
- Proposes a process/mechanism that provides ongoing oversight of the efforts covered by the action plan.
Examine the current composition of the Hartford Commission on Homelessness and if necessary make appropriate changes to facilitate the implementation process.
Measurable Outcome: The Mayor’s convening of the Hartford Commission on Homelessness.
Implementation Plan
Strategy 1. Create a subcommittee of the Commission (Planning Committee) to work on developing and launching Journey Home.
Strategy 2. Committee will ensure that critical activities occur and are not held up pending the establishment of Journey Home.
Strategy 3. Create governance and operational structure that will effectively carry out the implementation strategy.
Strategy 4. Create governance and operational structure that will effectively carry out the implementation strategy.
Strategy 5. Develop budgets that will enable Journey Home to effectively carry out the Implementation Plan.
Strategy 6. Raise the funds necessary to carry out the Implementation Plan.
