Goals:
- Implement the Greater Hartford Plan to End Chronic Homelessness
- Support the shelter, housing, and supportive service systems
- Create or replicate best practices through collaborative initiatives
- Measure progress towards ending homelessness
Vision:
That all persons facing homelessness in the Greater Hartford region will have access to safe, decent, affordable housing and the resources and supports needed to sustain it.
History:
- June 2004 – City of Hartford formed a project team to lay the foundation for the planning process to develop a plan to end homelessness in the Greater Hartford Area
- June 2005 – Ten-Year Plan to End Homelessness completed by the Mayor’s Commission to End Homelessness
- October 2007 – Implementation plan for the Ten-Year Plan to End Homelessness completed by Commission to End Homelessness with more than 150 stakeholders from the public, private, and non-profit sectors
- December 2007 – Journey Home, Inc. launched as an independent nonprofit organization to spearhead the implementation of the Ten-Year Plan to End Homelessness
- August 2008 – First Executive Director, Sarah Gallagher, hired
- December 2009 – Journey Home launched Capitol Region Homelessness Prevention and Rapid Re-housing Program
- May 2010 – Journey Home launched the Vulnerability Index Survey and begins tracking medically ‘vulnerable’ homeless
- January 2011 – Second Executive Director, Matthew Morgan, hired
- February 2011 – Journey Home launched the Moving On Initiative
